2012-2013 APPLICATION PROCESS
All interested teachers must submit their grant proposal to McDonald's^® prior to the April 4, 2012, deadline. A downloadable version of the application is available here. Each proposal must provide the information requested below and any other information you deem necessary. Applications should not exceed three typed, single-spaced pages printed on white paper. Six copies of each application which have been three-hole punched must be submitted for review by the panel. Please include:

1. Applicant's name.

2. School and school address.

3. Grade and estimated number of students.

4. Specific dollar amount requested.

5. A detailed description of the project and your learning objectives for the students.

6. A description of the circumstances which create a need for this project.

7. A description of how you will provide evidence of the project's effectiveness in terms of student progress through a quantitative evaluation process.

8. An itemized budget including: individual item costs, a total project cost, specific grant request and funding from other sources.

9. Signatures of the teacher and principal.

ELIGIBILITY
Any teacher, assigned to grades preschool through twelfth, with a classroom in the following Kansas counties is eligible to apply for a grant: Ellis, Graham, Gove, Lincoln, Logan, Ness, Osborne, Phillips, Rawlins, Rooks, Rush, Russell, Sheridan, Thomas or Trego.  Preschools must be associated with a Unified School District.  Teachers may apply for more than one grant, and those who have previously received grants may apply for funding for new projects.

Grant recipients will be expected to participate in public relations activities and may be asked to supply copies of their support items to the media. Recipients will need to submit a summary and evaluation of their project to McDonald's® by July 1, 2013. The summary should include: test results, exact costs, copies of support items and copies of local media coverage.
